Dole Fresh Vegetables (DFV) wants to consider transportation service providers that are capable of meeting our business and transportation requirements across our supply chain. This would include, but not be limited to consistent service on shipments in a timely manner, enabling and supporting the quality and freshness our Customers expect of every delivery we make to them. Below are the minimum requirements that all carriers must have to be eligible for consideration to become a contract transportation service provider for DFV. Carefully read over the below requirements and fill out the Carrier Registration Form.

• Carrier Requirements
      o Operating Authority for a minimum of two years with NO operating authority revocations during that time
      o FMCSA Safety Rating must not be Unsatisfactory or Conditional
      o Demonstrated current experience hauling Fresh Produce
      o 24/7/365 staffed customer support coverage
      o Smartway Partner
      o EDI Capabilities
Equipment Requirements
      o Refrigerated, 53’, swing door, Air Ride Trailer
      o Reefer Trailer equipment is no more than 5 years old
      o On demand visibility and access to information and data of tractor and trailer events
      o Trailer Refrigeration Unit (TRU) operating data/information must be downloadable on demand
      o Trailer Refrigeration Unit (TRU) must have the ability to be set up or already have the operating mode for Dole Fresh               Vegetables’ Fresh Protect Setting which has a 4°F delta from the set point (Set Point for DFV loads is 34 Continuous,             unless otherwise stipulated)
      o Door Sensor Technology referencing doors open/close + geographical location (GPS coordinates, City, State) is                     preferred
• Insurance Requirements
      o Commercial General Liability insurance $1,000,000 per occurrence $2,000,000 in aggregate. Dole Fresh Vegetables               must be named as Additional Insured.
      o Commercial Automobile Liability insurance $1,000,000. Dole Fresh Vegetables must be named as additional insured.
      o Workers Compensation Insurance $1,000,000
      o Cargo Insurance $100,000. Dole Fresh Vegetables must be named as Loss Payee
• Food Safety
      o DFV will provide each contracted carrier with a seal policy that must be followed for every shipment
      o All drivers who haul for Dole Fresh Vegetables must be made aware of the seal policy and understand the                               importance of trailer and seal control and integrity for every shipment while in the care and custody of the carrier.
      o DFV requires its contracted transportation carriers and brokers to comply with the criteria to meet the Sanitary                         Transportation requirements and regulations promulgated under the Food Safety Modernization Act (“FSMA”), and to             acknowledge their agreement to do so.
• All transportation service providers to be contracted with be required to review, understand, and agree to DFV’s Supplier Code of Conduct policies.
